Pregnenolone “The Mother Hormone”: Your compounding pharmacist’s opinion

​When monitoring a female patient’s hormone levels we hear the word pregnenolone very often. Just because we don’t see this hormone on paper a lot dosen’t mean we should not give it the attention it deserves. With a name like the mother hormone, we need to stop and give this molecule a little bit more attention and credit. 

​At the top of the hormone cascade,  pregnenolone is responsible for the conversion into DHEA, estrogen, progesterone, and testosterone...this is why it is referred to as the mother hormone. But where dose it come from? Pregnenolone actually comes from cholesterol which needs to be monitored closely when dealing with a patient with  low cholesterol levels. ​

When patients get to the age of 75, most people have a 65% decline in pregeneolone, compared to patients who are 35 years old. So what does that mean? If we don’t have  a sufficient amount of pregenenole in our bodies, we are not going to convert anything into estriol, estradiol, DHEA, and testosterone.
​Low pregnenolone will produce effects such as arthritis, depression, memory loss, fatigue, inability to deal with stress, insomnia, and lack of energy. The functions of pregnenolone are very important. Between regulation of stress and improving energy both physically and mentally, this hormone plays a tremendous part in regulating excitation and inhibition in the nervous system. This hormone also reduces pain and inflammation by modulating the NMDA receptor, which also plays a major role in memory.  
 
Understanding the role of pregnenolone in the hormone cascade is easy when you study the mechanism of the pregnenolone pull. This mechanism is better understood when looking at the sex hormone cascade on a piece of paper. When you are stressed, instead of your pregnenolone converting into your sex hormones it will convert into cortisol. This will cause menopausal symptoms. 

​As a compounding pharmacist at precision all I can do is give you my opinion. Monitoring pregnenolone should be done by a health care professional by drawing blood, saliva, or urine levels .The absorption of pregnenolone works best in a gelatin capsule with 40% methocel and a filler of your choice. The filler that I go with most of the time is avicel. The starting dose of pregnenolone is going to be 25mg. For any question please contact us for compounding advice.

​I have been working at Precision Compounding Pharmacy for nearly two years. This September I will be attending the University of Rhode Island in order to obtain my doctorate of pharmacy. ​
​ 
During my time at the pharmacy one of the most important medications I compounded and learned about was Porcine Thyroid. Thyroid capsules are used to treat hypothyroidism, which effects millions of people in the United States including some of my close friends. 
 
​Hypothyroidism is cause by a damaged and inflamed thyroid gland which prevents the body from receiving a sufficient amount of thyroid hormone. In order for patients to receive a healthy amount of the hormone they can take capsules to supplement what their body can not  produce.
 
One of the most common ways to treat hypothyroidism is through porcine thyroid. This is natural thyroid hormone extracted from pigs that imitates the hormone your body would naturally produce. ​
​It is becoming increasingly difficult and very expensive to receive porcine thyroid medication through retail pharmacies because the big producers, Nature Thyroid and WP Thyroid, are deep in backorder due to the high demand and are struggling to obtain the ingredients. Since we are a compounding pharmacy we are able to source the raw materials needed and compound the capsule with ease.  
 
Our porcine thyroid medication is made using veggie capsules that contain only natural fillers. Although our medication is not covered by insurance, it is far more affordable and readily available than if you were trying to get the same medication from a retail pharmacy.
